  4. Copy of Memorandum & Articles of Association & Incorporation.<\/li>\n\n\n\n
  5. Copy of National ID\/ Passport (Board of Director\/ Managing<\/li>\n\n\n\n
  6. Partners\/ Proprietor).<\/li>\n\n\n\n
  7. Copy of L\/C of Machinery\/ Commercial Invoice\/ Bill of Loading\/
    Bill of Entry.<\/li>\n\n\n\n
  8. List of Machineries (On Company Letter Head).<\/li>\n\n\n\n
  9. Copy of Fire License.<\/li>\n\n\n\n
  10. Copy of TIN Certificate.<\/li>\n\n\n\n
  11. Copy of Deed Agreement with land Owner \/ Land Ownership Document.<\/li>\n\n\n\n

    c) Machine: 11-20 = Tk. 25,000\/-
    d) Machine: 21-30 = Tk. 35,000\/-
    e) Machine: 31-40 = Tk. 45,000\/-
    f) Machine: 41-50 = Tk. 55,000\/-

  22. Group Insurance Fee pay to BGWIA as per declaration:
