7. Old Membership Certificate (Photocopy)
8. Any group Insurance Certificate (Photocopy)
9. Membership fee pay to BGWIA as per declaration :

1). Machine: 1-5= Tk. 12,000/-

2). Machine: 6-10= Tk. 18,000/-

3). Machine: 11-20= Tk. 25,000/-

4). Machine: 21-30 = Tk. 35,000/-

5). Machine: 31-40 = Tk. 45,000/-

6). Machine: 41-50 = Tk. 55,000/-

7). Machine: 51 above = Tk. 60,000/-

NB: Factory must be visited. In this case, the company will have to make travel arrangement.
